Paging on "All courses" is broken

After selecting "All courses" and then pressing page 2 button you get 404.
Link generated is:
pgcourses.php&ctype=all?start=20
? and & is mixed up

Hmm.... it no longer orders by course name when listing all, or when selecting by first letter
EDIT: sort=name should be default
