I can only now think that it ignores that its plugged on a fairway, although it could happen I suppose with soft fairways set ?

Its very similar to when you take a mulligan. The main difference is that when you take a mulligan you can reset the conditions back to the original shot by either aiming somewhere else or changing clubs. Neither of those two effect the plugged in the sand CTP shot tho.

For what it's worth, I have had some 2nd shots plugged in the fairway (and sand) even when not hitting the 1st shot into a bunker. It's fairly rare, but I remember those quirks that piss me off and that is one of them~!
